Privacy Notice

QRRX LLC, operating as QR Rx, runs Selesa.ai. This notice explains this public website's setup and access requests, browser storage and optional measurement. It supplements the QR Rx Privacy Policy.

1. Information we receive

When you send a request, we receive your name, email address, clinic name, whether you are requesting clinic setup or patient access, and any question you include. Clinic requests also include the selected specialty and practice website; a clinic website is optional for patient access requests.

We also process technical information needed to operate and protect the website and request service, such as IP address, browser information, verification results, request identifiers and server logs. Question and form entries stay in this tab until submission; this website does not put them into URLs or persistent browser storage.

2. How requests are used and shared

We use your request to respond, help with setup or access, keep a record of the inquiry and prevent abuse or duplicate submissions. Submitting a request asks the QR Rx team to follow up at the email address you provide; it does not subscribe you to unrelated marketing.

Requests are saved by QR Rx and may be handled by authorized team members through our support database, email and internal team tools. Service providers help us host the website, verify requests, store records and deliver notifications. We may also disclose information when required by law or to protect rights and security, as described in the QR Rx Privacy Policy. We do not sell inquiry information or use it for targeted advertising.

3. Optional Google Analytics

Google Analytics loads on the Selesa homepage only after you choose to allow analytics. It measures visits, setup starts, request receipts and errors, using broad source categories such as direct, Google or LinkedIn. It can use analytics cookies and process device, browser and network information, including an IP address during collection. See Google's explanation of Analytics data.

Our analytics events exclude your question, name, email, clinic details, form entries and request identifier. They use the homepage address rather than raw query strings or referrers. Advertising personalization, Google Signals and automatic form measurement are disabled for this site. A request receipt measures confirmation from our request service, not email delivery or a clinical response.

You can decline analytics and still use the form. To change or withdraw your choice, return to the homepage and select Privacy Preferences in the footer. Withdrawal stops future measurement; it does not erase data already collected. Global Privacy Control and Do Not Track browser signals keep optional analytics off.

4. Browser storage and security verification

Browser local storage remembers your analytics choice and display theme. You can remove these preferences by clearing this site's browser data. After analytics opt-in, Google may set analytics cookies; turning analytics off does not itself remove existing cookies. This site's optional analytics does not run on these policy pages or the clinic's patient care-plan pages.

Opening the request form loads Cloudflare Turnstile to help distinguish visitors from bots. This verification is separate from optional analytics and may process technical signals, including your IP address, browser information and the website origin. Read the Cloudflare Turnstile Privacy Addendum. Declining analytics does not disable this form-security check.

5. Retention and your choices

Inquiry records follow the contact-request retention described in the QR Rx Privacy Policy: noncustomer requests are kept for up to two years after the last contact, and message text follows that policy's deletion period of 90 days after a request closes. Retention can differ when a request becomes a customer relationship or when records are needed for security or legal obligations. A limited coded opt-out record may be kept to honor a request not to contact you.

You can ask for access, correction or deletion of eligible inquiry information, or ask us to stop contacting you, at privacy@qrrx.io. We may need to verify your identity. Applicable rights, exceptions and the request process are described in the QR Rx Privacy Policy.

6. Selesa within your clinic's care plan

Selesa in a clinic-provided QR Rx care plan is a separate experience from this public inquiry form. The QR Rx Privacy Policy, the clinic's Notice of Privacy Practices and the applicable Business Associate Agreement describe health-information handling. Contact your treating clinic about your medical record, care or a patient access link. Allowing website analytics is not authorization to disclose health information.

This public website is not directed to children under 13. A parent or guardian can request access help without including a child's medical information.

7. Changes and contact
