QRRX LLC, operating as QR Rx, provides this public Selesa website. These terms cover browsing Selesa.ai and submitting setup or access inquiries. The Selesa Privacy Notice explains how information from this website is handled.
A request is an inquiry. Submitting this form does not create a QR Rx account, enroll you in a paid subscription, sign a clinic Business Associate Agreement or confirm access to a patient care plan.
The question examples introduce Selesa. This website forwards a submitted inquiry to the QR Rx team and does not provide a medical answer.
1. Setup and access requests
Provide accurate contact and clinic information and submit only information you are authorized to share. By sending an inquiry, you ask the QR Rx team to contact you about that request. An on-screen receipt confirms that the request service received your inquiry; it does not guarantee email delivery, a response time or approval of access.
For patient access, use the link and instructions supplied by your clinic. Do not share access PINs, credentials or private care-plan links through this public form. Clinic setup and any paid services are subject to the plan details and agreements presented through QR Rx.
2. Clinical questions and urgent care
This public website is not a clinical consultation, diagnostic service or emergency-monitoring channel. Keep patient records and details about a patient's condition out of your inquiry. Contact your treating clinic for medical questions, medication instructions or concerns about recovery. For an emergency, call your local emergency number.
Selesa within a clinic's QR Rx care plan helps patients find clinic-approved information. It does not replace the treating team's judgment. The clinic remains responsible for reviewing and approving its patient instructions.
3. Responsible website use
Do not impersonate someone, send spam or malicious content, submit another person's information without authority, or interfere with verification, security or availability. Report suspected unauthorized access or a security issue to security@qrrx.io.
QRRX LLC and its licensors retain their rights in this website, software, design, content and branding. Browsing or submitting a request does not transfer those rights or grant access to the software, another clinic's data or patient information.
4. Information and availability
Website information and features may change as Selesa and QR Rx develop. Website examples do not guarantee a clinical outcome, accuracy for an individual patient or availability of every feature. If the request form is unavailable, you can email the QR Rx team about setup or access, keeping patient information out of your message.
5. QR Rx platform agreements
Use of the QR Rx platform, including Selesa in a clinic-provided care plan, is governed by the applicable QR Rx Terms of Service, Privacy Policy, order form and, when QR Rx handles health information for a healthcare customer, the Business Associate Agreement. Required agreement acceptance occurs through QR Rx's agreement-review process.
These public website terms do not amend an existing clinic agreement or BAA. The BAA controls conflicting provisions about protected health information. A website request or optional analytics choice does not replace required platform acceptance or patient authorization.
